A worker death on a northeast Missouri dairy farm last September leads to citations from the U.S. Department of Labor.

An investigation by the Occupational Safety and Health Administration cited eight violations in the death of a maintenance worker at a LaBelle dairy farm owned by Bethel Missouri based Sharpe Holdings. A worker was performing maintenance work  on an overhead door pulley when he fell from a 12 foot ladder onto a concrete floor.

OSHA proposes a 54-thousand dollar fine. Sharpe Holdings is contesting the findings.
